Two days after successful and uncomplicated aorto-coronary bypass surgery, the following ECG is recorded:

- Sinus rhythm, 70/min
- Right bundle branch block (RBBB), left anterior fascicular block (LAFB)
- RBBB + LAFB (bifascicular block)
In RBBB, the first 60 msec portion of the QRS reflects left ventricular activation. The presence of a left fascicular block can therefore be diagnosed:
