This is a small deflection that is noted after the QRS complex (late potential), predominantly in leads V1 and V2 and is indicative of late myocardial activation of the right ventricle in patients with arrhythmogenic right ventricular cardiomyopathy (ARVC). Because ARVC results in fibrofatty replacement of healthy myocardium, particularly in the right ventricle, regions of myocardium that are surrounded by fibrofatty tissue may depolarize later than the remainder of the myocardium and result in these late potentials.
