An 88-year-old man is brought to the Emergency Deptartment from a local nursing home because of an episode of syncope lasting one minute. Recently, he had been complainng of fatigue, and his pulse had been noted to be slow:

- Sinus rhythm with second degree AV block and ventricular escape complexes (escape-capture bigeminy)
- Ventricular rate 39/min
