These occur when the rate of the supraventricular impulses (atrial or junctional) is slower than that of the ventricular impulses. There may be single or multiple escape ventricular complexes (three or more complexes constitute a “ventricular escape rhythm”. The QRS complexes are usually wide, with a bundle branch block or nonspecific intraventricular block pattern. The rate is slow (usually 30-40/min, occasionally 20-50/min)
