History
This 83-year-old woman is brought to the Emergency Department from her nursing home after a brief episode of syncope:
Diagnosis
- Sinus rhythm with 2:1 AV block – ventricular rate 59/min, PR markedly prolonged (436 msec)
- Right bundle branch block

The patient is admitted to the cardiac ward for further monitoring and treatment. The following ECG is recorded six hours later:

- Ventricular rate 79/min
- Right bundle branch block
