A 49-year-old woman is in the cardiac clinic for follow up 3 months after aorto-coronary bypass surgery:
Diagnosis
- Coarse atrial fibrillation, with ventricular rate 86/min
- Inferior infarction, age undetermined
In atrial flutter saw-tooth waves are usually seen in the inferior leads, and are constant in pattern through the 10sec. rhythm strip. In this ECG tracing flutter-like waves are seen in V1, with changing rate and morphology, while in the inferior leads there are fine fibrillatory waves. The QRS rhythm is irregularly irregular. This is an example of coarse atrial fibrillation.
