History
A 44-year-old woman with familial dilated cardiomyopathy is in the Emergency Department with palpitation and severe dyspnea of one hour duration:
Diagnosis
- Ventricular tachycardia, 156/min
ECG findings in favour of the diagnosis of VT:
- Wide QRS tachycardia with QRS duration >140 msec.
- AV dissociation
- RBBB pattern with broad monophasic R waves in V1 and QS in V6
The clinical presentation of a wide QRS tachycardia occurring suddenly with severe symptoms in a patient with known cardiomyopathy strongly favours VT.
