History
A 70-year-old woman with chronic severe obstructive lung disease is in the medical intensive care unit with bronchopneumonia:
Diagnosis
- Atrial tachycardia with 2:1 AV block
- Ventricular rate 96/min
- Right axis deviation
- Right ventricular hypertrophy
- Non-specific T wave abnormality
The episode of atrial tachycardia occurred shortly after admission to ICU and lasted approximately 12 hours. The echocardiogram confirmed the presence of pulmonary hypertension. (PA pressure 65 mmHg)

Compared with the previous tracing:
- Sinus rhythm, 98/min with one premature atrial complex has replaced atrial tachycardia with 2:1 AV block
- Right axis deviation
- Right ventricular hypertrophy with repolarization abnormality
