This type of block occurs in patients who have disease of the His Purkinje system such that current leak within the cells occurs during phase IV of the action potential. As a result, when a significant delay has occurred between QRS complexes, the slow leak may cause partial depolarization of the His Purkinje and result in a beat that is conducted with block in one or more fascicles.
