History
A 56-year-old woman has been suffering for the past 20 year from occasional episodes of rapid regular palpitation, starting abruptly, lasting from a few minutes to half an hours, accompanied by sensation of pulsation in her neck. She is now in the Emergency Department with one such episode that started 45 minuted earlier:
Diagnosis
- Supraventricular tachycardia, 182/min
- AV Nodal Re-entrant Tachycardia (AVNRT)
- Non-specific ST abnormality
ECG 2
Second ECG is recorded 13 minutes later. Carotid sinus massage has been successful:
ECG 2 Diagnosis
- Sinus rhythm, 58/min
- ST segment normal
Notice: the pseudo-r’ is no longer present in V1
Transient ST depression frequently occurs during episodes of paroxysmal SVT, even in the absence of other cardiac abnormalities.
