Case 151: A 52-Year-Old Man who Fell

This 52-year-old man was seen in the Emergency Department after an accidental fall. Investigation for head trauma was negative.
A routine ECG was obtained.
Diagnosis
- Dextrocardia
The patient’s dextrocardia had incidentally been discovered after a chest X-ray in his twenties. Further assessment did not demonstrate any other cardiac abnormalities. The ECG findings are typical of dextrocardia with situs inversus: marked right axis deviation, negative P waves in I and aVL and positive P waves in aVR, and rS pattern in the chest leads with decreasing low voltage in V4 to V6.
