Case 5: A 78 Year-Old Man in the Defibrillator Clinic

A 78 year old man with a primary prevention implanted defibrillator (ICD) presents to the defibrillator clinic for routine follow-up. His interrogation reveals an episode which is shown below:
Diagnosis
- Sinus tachycardia, 135/min
- Inappropriate Anti-tachycardia Pacing
Comment: Note that during sinus tachycardia (this may be an atrial tachycardia if the patient was not exerting himself at the time) some of the atrial electrograms are not sensed appropriately (marked by “X”) and thus the defibrillator diagnosis the arrhythmia as ventricular tachycardia because there are “more V’s than A’s”.
