This 73-year-old woman presents to clinic postoperative day two from the replacement of her mitral valve due to severe mitral regurgitation:
Diagnosis
- Sinus rhythm with Mobitz I second degree AV Block
- Low voltage QRS
- Poor R wave progression from V1 to V3 (consider the possibility of septal myocardial infarction)
- Non-specific T wave abnormality
Poor R wave progression from V1 to V3 had been noted in the past, and coronary angiography prior to surgery had been normal.
