Case 7: A 75-Year-Old Man in the CCU

History
This 75-year-old man is brought to the hospital by ambulance and admitted to the CCU 45 minutes after the onset of severe anterior chest pain, weakness and dyspnea:
Diagnosis
- Sinus tachycardia, 106/min
- Left ventricular hypertrophy
- ST elevation in aVR and V1, and marked diffuse ST depression suggestive of main left coronary occlusion
- Acute Myocardial Infarction
The patient was transferred to the cath lab for emergent angiography, which demonstrated occlusion of the main left and diffuse moderate disease of the right coronary artery. The procedure was complicated by cardiogenic shock, and an intra-aortic balloon pump was inserted.
