History
This 81-year-old woman just underwent surgery for small bowel obstruction. 15 years ago, she suffered from a myocardial infarction, requiring a 2-vessel aorto-coronary bypass.
She is presently in the post-anaesthetic care unit, and has developped the following arrhythmia:
Diagnosis
- Ventricular tachycardia, 133/min with one fusion complex
In favor of ventricular tachycardia:
– Wide QRS complex (140 msec)
– AV dissociation (presence of fusion complex)
– Negative QRS concordance in the chest leads

The patient received IV Amiodarone, and the arrhythmia subsided. Her condition remained stable:
ECG 2 Diagnosis
- Sinus rhythm with sinus arrhythmia and occasional premature supraventricular and ventricular complexes
- Low voltage QRS
- Possible anterior infarction, age undetermined
- Non-specific T wave abnormality
