Case 5: A 69-Year-Old Man in the Cardiac Clinic

This 69-year-old man presents to the cardiac clinic for his yearly assessment. Seven years ago, he suffered from a myocardial infarction that was treated successfully with angioplasty. He has no cardiac complaints, and is able to do moderately heavy exercise without discomfort:
Diagnosis
- Sinus rhythm, 80/min
- Previous lateral infarct – note the Q waves in leads I and aVL
- Tall R wave in V2, consider posterior infarct
At the time of his myocardial infarction seven years ago, coronary angiography revealed occlusion of the right and circumflex coronary arteries.
