History
A 43-year-old man is seen in the pre-admission clinic prior to his upcoming thyroidectomy, and a routine ECG is performed:
Diagnosis
- Dextrocardia
- Likely Sinus Rhythm
- Premature Ventricular Complex
The negative P-wave and QRS complex in lead I along with the R wave regression across the lateral precordial leads should be a clue towards the presence of dextrocardia. Sinus rhythm can be confirmed once the leads are placed correctly (on opposite sides of the torso).
