This 40-year-old man with a known patent ductus arteriosus presents with increasing dyspnea on exertion over the past five years:
Diagnosis
- Sinus rhythm
- Right atrial enlargement
- Right bundle branch block plus right ventricular hypertrophy (RBBB and RVH) with repolarization abnormality
This patient has Eisenmenger syndrome. His condition has severely deteriorated in the recent months, and he is being investigated for possible heart-lung transplant. His ECG shows right bundle branch block plus right ventricular hypertrophy (RBBB, right axis deviation, R1 in V1 >15 mm.), with marked ST and T changes due to the conduction abnormality and RVH.
