A 88-year-old woman is admitted to the neurology ward from a nursing home for recent onset of confusion and left-sided weakness:

• The left leg electrode/cable is detached/non-functional
• This precludes the interpretation of the limb leads
• Non-specific ST depression in V5-V6, possibly due to baseline shift (see 10 sec V5 strip)

Please, obtain a new ECG with a different set of electrodes/cables.

A tracing is obtained 15 minutes later with a different set of electrode/cables:   

• Unusual P axis, consider ectopic atrial bradycardia
