Case 42: A 52-Year-Old Man with Recent Hospital Admission

A 52-year-old man admitted 1 week earlier with retrosternal chest pain that lasted about one hour and mild troponin elevation is referred for further investigation and treatment:
Diagnosis
- Sinus rhythm, 82/min
- Lateral myocardial infarction
Coronary angiography demonstrated a total occlusion of the marginal branch of the circumflex artery and a 40-50% stenosis of middle segment of the LAD.
Angioplasty of the circumflex artery was unsuccessful, as the occlusion could not be crossed. There were no complications. The LAD lesion was moderate and it was felt it did not warrant angioplasty.
