A 47-year-old woman with known cardiomyopathy, is in the Intensive Care unit in low-output circulatory failure with recurrent episodes of tachyarrhythmias:
Diagnosis
- Polymorphic ventricular tachycardia
Polymorphic ventricular tachycardia, rate 180/min.The QRS is markedly wide (180 msec) with variable pattern (best seen in the V1 and II rhythm strips).
There is a negative QRS pattern from V1 to V6 (negative concordance). During the last second of recording the ventricular tachycardia ends with return to the previous rhythm (atrial fibrillation with non-specific intraventricular block).
