A 42-year-old diabetic man is seen by his family physician because of three episodes of anterior chest discomfort and breathlessness  during the past two months:

• Sinus rhythm
• Left atrial enlargement
• Lateral myocardial infarction, age undetermined
• ST and T abnormality, consider inferior ischemia
