A 68-year-old man is admitted to the coronary care unit with severe anterior chest pain:

• Sinus rhythm
• Right bundle branch block
• Acute anterior myocardial infarction
• Lateral injury pattern

1 hours after admission an arrhythmia occurred, associated with dyspnea, weakness and hypotension:

• Ventricular Tachycardia (VT)
  

ECG features suggestive of VT:    

• Regular wide-QRS tachycardia (rate 175/min, QRS duration >140 msec)
• RBBB pattern, with broad monophasic R waves in V1, QS in V4-V6 (a pattern likely to be due VT, rather than to rate-related aberrancy)
•  Right-superior axis
