History
A 68-year-old man is admitted to the coronary care unit with severe anterior chest pain:
Diagnosis
- Sinus rhythm
- Right bundle branch block
- Acute anterior myocardial infarction
- Lateral injury pattern

1 hours after admission an arrhythmia occurred, associated with dyspnea, weakness and hypotension:
ECG 2 Diagnosis
ECG features suggestive of VT:
- Regular wide-QRS tachycardia (rate 175/min, QRS duration >140 msec)
- RBBB pattern, with broad monophasic R waves in V1, QS in V4-V6 (a pattern likely to be due VT, rather than to rate-related aberrancy)
- Right-superior axis
