Case 110: A 69-Year-Old Man with Sarcoidosis

A 69-year-old man, treated for systemic sarcoidosis for the past 2 years, is admitted for treatment of an arrhythmia:
Diagnosis
- Sinus rhythm with Mobitz I second degree AV block
- Ventricular rate 48/min
This patient had been known to have second degree AV block for several months and his heart rate had been in the 40-50/min range. He refused a pacemaker. Before his admission an episode of transient complete heart block associated with severe weakness had been documented.
A QS pattern is present in this tracing in V1 and V2, however the R wave in V3 measures 7 mm. The findings are likely due to misplacement of lead V2, as a repeat ECG recorded a few hours later showed normal R wave progression from V1 to V3, unchanged from previous tracings.
