A 47-year-old man is brought into the Emergency Department by ambulance, 45 minutes after the onset of severe retrosternal chest pain: 

• Sinus rhythm, 80/min
• Acute anterior myocardial infarction

Coronary angiography demonstrated an occlusion in the proximal segment of the left anterior descending artery.
